Before we begin, please make sure you have the following documents ready. You will need to upload your completed pdf, scans of your documents, or you can fill this form out from your phone and take a photo.

  • Tax Form - W-9 (Contractors) or W-4 (Employees)

  • Driver's License or Passport

  • If a licensed professional contractor, you'll also need to upload proof of Liability Insurance and your Business License

Read and acknowledge the following terms and conditions:

1. Independent Contractor Status

By signing this agreement, you acknowledge that you are an independent contractor and not an employee of Joy of Beauty® or its affiliated businesses. You retain full control over the acceptance and scheduling of assignments and are responsible for your own taxes, insurance, and business expenses.

2. Contractor-Affiliate Program

As a contractor, you are eligible to participate in our Affiliate Program to earn additional commission on product sales. When a guest requests to purchase any Joy of Beauty® products through you, you will receive a 15% commission on the sale. We will provide you with postcards featuring your custom affiliate link, ensuring you receive proper credit for referred purchases. 3. Compensation & Pay Schedule

Contractors receive a 60% commission, which includes any applicable travel fees. Payments are made twice monthly via direct deposit:

Work completed January 1 - 14 is paid on February 1.

Work completed January 15 - 31 is paid on February 15.

This cycle continues throughout the year.

4. Availability & Scheduling

As an independent contractor, you have the flexibility to accept or decline assignments at will.

Assignments are on an "as-needed" basis, meaning you will receive requests based on demand.

We require a three-day lead time for booking inquiries.

5. Termination & Modification of Agreement

Joy of Beauty® reserves the right to update, modify, or terminate this agreement at any time, with or without notice. Contractors who fail to meet the requirements outlined above may be removed from the program at our discretion.
